To schedule a visit, please fill out the Application Form below, in its entirety, and email to applications@projectwalk.org or fax to (760) 431-1598. We will notify you by email or phone when we receive it, so be sure to include your email address. This form must be completed and submitted to Project Walk prior to scheduling your visit.

The Client Application Form consists of eleven (11) pages with five different sections: Client Application, Service Agreement, Release of Liability, Billing Information and Skin Check. Make sure to fill out each page in its entirety.

Train Your Therapist/Trainer (TYT™) Supplemental Application
The TYT supplemental application should only be completed when the client has choosen to participate in a TYT program. This application is to help our staff plan for the visting trainer whether you are participating in the TYT Family Track or the TYT Professional Track. There are only two sections: TYT Application and Confidentiality Aggreement. Make sure to fill out each page in its entirety.
